Incident Report
Title IX of the Education Amendments of 1972 prohibits sexual harassment and sex-based discrimination in school programs or activities. If you report an incident of harassment or discrimination categorized as sex, sexual harassment, and/or sexual violence; sexual orientation; gender identity; or gender expression, the report and/or incident will go to the District Title IX Coordinator for review and follow-up.
(The District Title IX Coordinator and/or Deputy Title IX Coordinators will contact you to explain the formal Title IX grievance process, including the submission of a signed formal complaint. This form is considered a report and does not constitute a formal complaint.)
